The book examines the history and development of public administration, the study of the internal structure and functioning of government and its interaction with society and its citizens. It surveys different approaches to the field and the methodological and epistemological issues surrounding an interdisciplinary, applied social science.

We examined how children’s strategy choices in solving complex subtraction problems are related to grade and to variations in problem complexity. In two studies, third- and fifth-grade children (N≈160 each study) solved multi-digit subtraction problems (e.g., 34 - 18) and described their solution strategies. In the first experiment, strategy selection was investigated by means of a free-choice paradigm, whereas in the second study a discrete-choice approach was implemented. In both experiments, analyses of strategy repertoire indicated that third-grade children were more (...) likely to report less-efficient strategies (i.e., counting) and relied more on the right-to-left solution algorithm compared to fifth-grade children who more often used efficient memory-based retrieval and conceptually-based left-to-right (i.e., decomposition) strategies. Nevertheless, all strategies were reported or selected by both older and younger children and strategy use varied with problem complexity and presentation format for both age groups. These results supported the overlapping waves model of strategy development and provide detailed information about patterns of strategy choice on complex subtraction problems. (shrink)

The purpose of this article is to describe the development of a model of moral distress in military nursing. The model evolved through an analysis of the moral distress and military nursing literature, and the analysis of interview data obtained from US Army Nurse Corps officers (n = 13). Stories of moral distress (n = 10) given by the interview participants identified the process of the moral distress experience among military nurses and the dimensions of the military nursing moral distress (...) phenomenon. Models of both the process of military nursing moral distress and the phenomenon itself are proposed. Recommendations are made for the use of the military nursing moral distress models in future research studies and in interventions to ameliorate the experience of moral distress in crisis military deployments. BackgroundObtaining informed consent for intravenous thrombolysis in acute ischemic stroke can be challenging, and little is known about if and how the informed consent procedure is performed by neurologists in clinical practice. This study examines the procedure of informed consent for intravenous thrombolysis in acute ischemic stroke in high-volume stroke centers in the Netherlands.MethodsIn four high volume stroke centers, neurology residents and attending neurologists received an online questionnaire concerning informed consent for thrombolysis with tissue-type plasminogen activator (tPA). The respondents were (...) asked to report their usual informed consent practice for tPA treatment and their considerations on whether informed consent should be obtained.ResultsFrom the 203 invited clinicians, 50% (n = 101) completed the questionnaire. One-third of the neurology residents (n = 21) and 21% of the neurologists (n = 8) reported that they always obtain informed consent for tPA treatment. If a patient is not capable of providing informed consent, 30% of the residents (n = 19) reported that they start tPA treatment without informed consent. In these circumstances, 53% of the neurologists (n = 20) reported that the resident under their supervision would start tPA treatment without informed consent. Most neurologists (n = 21; 55%) and neurology residents (n = 45; 72%) obtained informed consent within one minute. None of the respondents used more than five minutes for informed consent. Important themes regarding obtaining informed consent for treatment were patients’ capacity, and medical, ethical and legal considerations.ConclusionThe current practice of informed consent for thrombolysis in acute ischemic stroke varies among neurologists and neurology residents. If informed consent is obtained, most clinicians stated to obtain informed consent within one minute. In the future, a shortened information provision process may be applied, making a shift from informed consent to informed refusal, while still considering the patient’s capacity, stroke severity, and possible treatment delays. In Rogers, the Court of Appeal held that the decision of Swindon N.H.S. Primary Care Trust to refuse to fund Herceptin for the treatment of Ann Rogers against breast cancer was irrational. The P.C.T. maintained that their decision was not resource driven but based on the fact that Herceptin was, at that time, not licensed by the European Medicines Agency (E.M.E.A.) for use in early stage breast cancer. Yet it was prepared to fund its use in ‹exceptional circumstances’ which could (...) only be based upon the personal situation of the patient. In this note, I focus upon the ‹exceptionality’ argument in the context of the patient as consumer of healthcare and women as carers. I argue for a focus upon caring rather than consumerism in healthcare. (shrink)

Transsexualism, a condition in which from earliest recollection the individual is unshakeably convinced he or she has been endowed with the wrong physical sexual body presents one of the most difficult problems as regards management and treatment in clinical medicine. The aetiology and treatment is discussed and the problems with which the medical practitioner may be faced in advising his transsexual patient which include change in social role and marriage and the difference between sex and gender.

ABSTRACTDiffraction gratings are famously associated with Henry Rowland of Johns Hopkins University but there were precursors. Although gratings were first made and used in Europe, reliable machines for ruling gratings were developed in the USA, and two men, Lewis Rutherfurd and William Rogers, tackled the problem before Rowland. Rutherfurd, a wealthy independent astronomer, designed and built the first screw-operated engine for ruling diffraction gratings, the fore-runner of almost all subsequent ruling engines. With it he and his assistant D. C. Chapman (...) ruled many gratings which he generously distributed to practising scientists, thereby materially advancing the science of spectroscopy. Rogers was a Harvard astronomer who developed an interest in the ruling of fine lines on glass that led him to construct a ruling engine with which he investigated the causes of the errors in the rulings he had examined. He continued to seek improvements with a second engine designed for ruling diffraction gratings. He ceased developing this engine when Rowland’s excellent gratings began to be available, concentrating instead on related problems to which he could apply the knowledge and skills he had gained, but his investigations assisted Rowland and other later ruling engine builders. This paper brings together what is known about the ruling engines of Rutherfurd and Rogers, their development, the gratings they produced, their quality and the work that was done with them, and assesses and compares their achievements and the impacts of the work of these two men. SummaryDiffraction gratings have contributed enormously to modern science. Although some historians have written about them, there is much more to be brought to light. This paper discusses their development and use in the period up to about 1880 before Rowland began to produce them. Rittenhouse described the action of a diffraction grating in 1786, but no explanation was possible until the wave theory of light was developed. Fraunhofer discovered the dark lines in the solar spectrum in 1814, and then investigated (...) diffraction, producing the first ruled gratings, making detailed measurements and calculating the wavelengths of prominent spectral lines. After Bunsen and Kirchhoff showed the association between spectral lines and chemical elements there was an upsurge of interest in measuring wavelengths. The gratings used in this work almost all came from one source, a relatively unknown instrument maker called Nobert, who made them by an extremely laborious process using a machine he had built himself. The most significant wavelength measurements were made by Ångström, but Mascart, Van der Willigen, Stefan, Ditscheiner and Cornu also did important work. Nobert gratings were investigated by Quincke, copied photographically by Rayleigh, and were known and discussed in the USA. Nobert's work helped to advance spectroscopy much more than has been acknowledged. (shrink)

The objective of this work is to categorize the available fly ashes in different parts of the world into distinct groups based on its compositional attributes. Kohonen's self-organizing feature map and radial basis function networks are applied in a cascading fashion for the classification of fly ashes in terms of its chemical parameters. The basic procedure of the methodology consists of three stages: apply self-organizing neural net to ascertain possible number of groups, delineate them and identify the group sensitive attributes; (...) find mean values of sensitive attributes of the elicited groups and augment them as start-up prototypes in k-means algorithm and find the refined centroids of these groups; incorporate the centroids in a two layer radial basis function network and fine-tune the delineated groups and develop an indexing equation using the weights of the stabilized network. Further, to demonstrate the utility of this classification scheme, the so formed groups were correlated with their performance in High Volume Fly Ash Concrete System [HVFAC]. The categorization was found to be excellent and compares well with Canadian Standard Association's [CSA A 3000] classification scheme. (shrink)

ABSTRACT During a visit to Europe in the autumn of 1882, Henry Augustus Rowland, Professor of Physics at Johns Hopkins University, displayed diffraction gratings produced on a ruling engine he had designed and built, which were bigger and much higher quality than any previously made. Some were of a novel type, ruled on concave surfaces, which he used in a simple but equally novel spectroscope that he had devised, to reveal spectral lines in great detail, and by means of photography (...) to record spectral data much more rapidly than previously possible. Over about twenty years Rowland built three ruling engines, published photographic maps of the solar spectrum, compiled a catalogue of the wavelengths of lines in the solar spectrum correlated with laboratory-produced spectra of almost all the chemical elements, and produced and sold the diffraction gratings used by spectroscopists everywhere. For decades after his death Rowland’s ruling engines remained practically the only source of good-quality diffraction gratings. This paper describes and analyses this work of Rowland and of the other men, Theodore Schneider, John Brashear, and Lewis Jewell, who played major roles in it. (shrink)

In “Two Concepts of Liberty” Berlin notes the protean nature of the word “freedom” and then systematically proceeds to narrow its range of meanings. In the process, Berlin eliminates much of what most people, in everyday communication, regard as freedom, believing that this is in the best interest of intellectual clarity. As he puts it: [N]othing is gained by a confusion of terms. To avoid glaring inequality or widespread misery I am ready to sacrifice some, or all, of my freedom: (...) I may do so willingly and freely: but it is freedom that I am giving up for the sake of justice or equality or the love of my fellow men. I should be guilt‐stricken, and rightly so, if I were not, in some circumstances, ready to make this sacrifice. But a sacrifice is not an increase in what is being sacrificed, namely freedom, however great the moral need or the compensation for it. Everything is what it is: liberty is liberty, not equality or fairness or justice or culture, or human happiness or a quiet conscience. (Emphasis added). Berlin's other, perhaps overarching, aim is to show how inattention to the specificity of the meanings of concepts might have potentially dangerous political repercussions. He implicitly argues that the intellectuals who promoted the idea of positive freedom as opposed to that of negative freedom contributed to the emergence of totalitarianism and fascism in Europe.
